Cal Poly’s Commencement includes multiple ceremonies, all of which share one official program listing the names of eligible graduates.
To ensure your name is included, please review the following deadlines and complete the form accordingly:
- April 22, 2026 - Submit by this date to be included in the printed program. Submissions received after this deadline may not appear in the printed version.
- May 29, 2026 - Final deadline to be included in the digital program.
If you meet one of the conditions below, please complete the form by the appropriate deadline.
This form is NOT REQUIRED if your expected graduation term is listed in your portal as Fall 2025, Winter Quarter 2026, Spring Quarter 2026 or Summer Quarter 2026. Learn how to check your expected graduation term.
Please only complete this form if:
- You are an undergraduate student and your expected graduation term is not listed above.
- You are an undergraduate student and recently changed your graduation term.
- You would like to participate in a commencement ceremony different from the one you are automatically assigned to.
