Family Input Survey: CLTS Waiver Renewal (2025)

The Wisconsin’s Department of Health Services (DHS) is starting the process to renew the Children’s Long-Term Support (CLTS) Program waiver for 2027-2031. The CLTS Program waiver describes how the CLTS Program works, who is eligible, what services are available, and how families can direct their own services. Renewing the CLTS Program waiver is a chance for DHS to review and improve the program. The first step in the renewal is to gather feedback from participants, their families, and others who interact with the CLTS Program. 

This survey asks families to share feedback about their CLTS Program experience. Responses will help DHS know what to focus on to improve the program over the next five years. If you have any questions or want to provide additional feedback, please email at DHSCLTSWaiverRenewal@dhs.wisconsin.gov

The survey should take about 10-15 minutes. It has two parts: First, we ask you to rate the importance of five possible waiver changes. Second, we ask you four open-ended questions.
