Please note: you may receive a follow up email from Cure SMA if additional information is needed to determine eligibility and process your application. Failure to reply to this email in a timely manner could delay your applications review.
Applications will be reviewed for eligibility and requests will be awarded as funding allows with both children and adults with SMA being eligible.
If approved, you will be required to complete a separate online equipment waiver prior to receiving the wheelchair.
Background Information
Thanks to generous funding from the Luke 18:1 Foundation, individuals with SMA can apply to receive a brand new Fold & Go wheelchair through the Fold & Go Equipment grant program and it will be theirs to keep if they are approved. This is a new program and unlike our current equipment pool loan program where most items are expected to be returned.
