The primary goal of the FASD United Affiliate Network is to unite organizations, associations, tribes, and other entities that address FASD into a national and international network. The network functions as a coalition, and each affiliate organization maintains its independence and identity. FASD United is committed to supporting the Affiliate Network in meeting this goal.
This evaluation is intended to be filled out by ONE representative at your organization. If you have multiple staff who engage with FASD United and the Affiliate Network, please coordinate with them to determine who will be filling out the form.
The purpose of this survey is not in any way intended to evaluate individual Affiliate member organizations, but to understand the functions, effectiveness and goals of the Affiliate Network as a whole. Your responses provide an opportunity for FASD United to understand what we are doing well and where we can adapt to better meet the needs of the Affiliate Network.
Please be honest in your responses, so we can best mold the Network to support Affiliates’ changing needs.This initial survey will serve as a baseline.
