About This Survey
This survey should take approximately 15 minutes to complete. It is being conducted solely for research purposes.
All responses are completely confidential—no names or identifying information will be shared with the University of Rochester or included in any reporting.

Opportunity to Join an Online Interview Session
At the end of the survey, you’ll have the opportunity to sign up for a virtual focus group. If you are chosen and you complete the session, OHO will send you a $50.00 Amazon Gift Card.

  • Dates: Sessions will be held between Wednesday, August 27 - Wednesday, September 3, 2025.
