Corewell Health Healthier Communities Grant Multi-Year Fund FY 2026 - 2027

Funding Award Period: January 1, 2026 – December 31, 2027
                                                                                                                                            
Corewell Health Healthier Communities is deeply committed to supporting the work of community organizations in Kent County to transform the conditions that shape our community’s health. We are looking to support impactful projects that align with our Kent County, MI Community Health Needs Assessment. We seek to award eligible nonprofit organizations that address and improve outcomes in at least one of the following strategic priority areas:

Access to Care: Ability to get to, receive, and use all aspects

Award Amount and Timeline
Organizations can apply for awards up to $150,000. The total award amount will be distributed over a two-year period. Please note: Award distribution in Year 1 will be greater than the distribution in Year 2. Collaborative proposals, if successful, will be awarded up to $200,000 total between both organizations.

Submit a Donation Acknowledge Letter within 30-days of receiving the distribution.
**Grantee reports and reporting requirements must be met.
 

Eligibility

  • 501(c)3 organization or a project with a fiscal sponsor, or a government agency
  • Must provide services or programs for underserved and economically disadvantaged populations in rural and/or urban communities of Kent County, MI
  • Must have attended a virtual grant info session on July 29 or 31, 2025
     

Types of Funding - These are non-discretionary funds which can be used for any purpose, including non-program/project activities.

  • Existing projects: a project that has years of implementation or is ongoing
  • Pilot projects: a project in its infancy seeking to assess or prove its viability
  • Capacity-building projects: a project that builds infrastructure in organizations or communities
  • Operational support, including staffing or operational costs
  • Projects that affect system/community level change, i.e. policy change(s), collaboration initiatives, sustainability projects, impact projects, etc.
Reporting
  • Mid-year site visits (on-site) will be conducted with a Healthier Communities Team member to review your outcomes, budget, successes, and areas of opportunity
  • Mid-year reports: July 31, 2026 and July 31, 2027
  • Year-end reports:  January 31, 2027 and January 31, 2028
