Thank you for agreeing to complete this questionnaire.  Your candid responses and comments on the questionnaire itself are most appreciated.  Please take notes as you go through this questionnaire so that if any question does not work for you in some way, you can indicate this at the end of the questionnaire.  You will be asked to provide your comments then.  Remember, there are no right or wrong answers.

 
CONSENT TO PARTICIPATE IN RESEARCH

You are being invited to participate in a study to investigate your attitudes and perspectives. This project is being conducted under the direction of Dr. Jean Kantambu Latting, Professor Emerita of Leadership and Change in the College of Social Work at the University of Houston.

PURPOSE OF THE STUDY
This study measures individuals’ attitudes and behaviors in community, professional, and work settings for the purposes of increasing knowledge about leadership development generally. The survey is conducted online and will take approximately 20-25 minutes to complete.

VOLUNTARY NATURE OF PARTICIPATION
Your participation in this survey is voluntary. If you decide not to participate in the study, or withdraw at any time, you will not be penalized in any way. You can also refuse to answer any question in the survey without penalty or consequence.

PROCEDURES
The survey contains questions about your attitudes toward different aspects of person and group dynamics and how you typically respond in situations. Please answer candidly; there are no right or wrong answers. 

The survey asks for some demographic information such as your job classification, educational level, and gender. These questions are asked so that trends in responses can be identified.

CONFIDENTIALITY AND ANONYMITY
Your individual responses will be confidential, known only to the researchers. We are interested in group averages, not individual responses. You will not be asked to give your name. You will be asked to register for the survey using an e-mail address and password, so that if you cannot complete the survey in one sitting, you can log back in to finish. Your e-mail address will be stored separately from your data responses in a location that is password protected. No information regarding IP addresses will be collected.

You may use any active e-mail address. If you wish to ensure that you remain completely anonymous, create an e-mail address using a free public account (such as Yahoo or G Mail) that cannot be connected with you. Please record the e-mail address and password used for this survey in a place where you can easily find it again; this will allow you to log off and on at your convenience. If you need any help with your account, e.g., resetting your password, you will be asked to give your e-mail address.

Your responses to the questionnaire and your scores will be stored on our database server, but will only be used anonymously in ongoing research by Dr. Latting and her colleagues.

BENEFITS
Your participation will help contribute to the research on how to develop leaders in community and work settings.

Once you’ve completed the survey, you may receive your scores on various subsets of the survey. This will allow you to see how your scores compare to those of others who have responded to the same questions. You may choose not to answer any question, but doing so will prevent us from being able to provide your scores on those scales.

RISKS
There are no known risks in participating in this survey.

REPORTS AND PUBLICATIONS
The results of this study may be published in professional and/or scientific journals. The results may also be used for educational purposes or professional presentations. No individual subject will be identified.

WHAT WE SHARE
In some cases, other researchers have allowed us to use their questionnaires on this website. In return, we have agreed to share your responses with them for their own research. The researchers will only receive your survey responses and demographic information. They will not receive your e-mail address.

We contract with other organizations for the development and maintenance of the website. This may require access to the data collected, for the purpose of assisting in the operation of the website. These organizations are bound by strict confidentiality requirements.

QUESTIONS AND CONCERNS
Any questions regarding your rights as a research subject may be addressed to the University of Houston Committee for the Protection of Human Subjects (713-743-9204). All research projects that are carried out by investigators at the University of Houston are governed by requirements of the university and the federal government.
