Welcome to the 2025 Edison Township Community Health Needs Assessment survey.
This survey will help guide efforts to address health challenges and create meaningful solutions for the needs of your community. The survey should be completed in one session. The information will help us:
• Understand challenges that affect our community
• Better understand the needs of our community
• Work together to find solutions to our needs
Surveys are anonymous and your answers will be private. We will not collect your personal information and we will not share how you answered the survey with anyone.
In order to complete the survey, you must be at least 18 years old.
