Multi-Unit Dwelling (MUD) EV Charging Program Incentive Redemption Form
Thank you for submitting the required information and documentation to process your MUD EV Charging Program incentive.

Please have the following information available before proceeding with this form.

Your application number.
Proof of purchase for the incentivized charging station(s) (with date, make/model, and costs of charging station(s) visible).
Site photos of the completed project.
Make, model, and serial numbers for all incentivized charging stations.
Utility bill, if redeeming for the make-ready costs in a non-IOU territory.
A copy of your network provider agreement.
Itemized breakdown of make-ready costs, if applicable.
