North Carolina Children and Youth with Special Health Care Needs (CYSHCN) Strategic Plan Needs Assessment Survey

On behalf of the North Carolina Department of Health and Human Services (NCDHHS), you are invited to respond to this survey as part of the needs assessment process for the upcoming North Carolina Children and Youth with Special Health Care Needs (CYSHCN) Strategic Plan. Your valuable input will directly inform the development of strategies and priorities aimed at improving the lives of CYSHCN and their families across our state.
This survey is structured around the principles and strategies of the American Academy of Pediatrics’ "Blueprint for Change," focusing on three critical areas:
  1. Family & Child Well-being and Quality of Life: How the system of services helps CYSHCN and their families have a better quality of life, feel supported, and grow in healthy and positive ways.
  2. Access to Services: CYSHCN and their families get the care and help they need, when they need it. This includes doctors, dentists, mental health providers, help at home or in the community, and people who help coordinate care as the child grows up.
  3. Financing of Services: Health care and other services are easy to get, not too expensive, cover all needs, and keep going without big gaps. They focus on helping CYSHCN and their families stay healthy and well.
Your responses will help us understand current strengths, identify gaps, and guide resource allocation and program planning for the next three years.
