FACES - Strategic Plan Electronic Survey

Since the 1960's, FACES: The National Craniofacial Association has served children and adults with severe craniofacial differences by offering financial travel assistance, raising public awareness, and providing information and resources for families.

FACES strives to provide services that meet the greatest need. As needs change, so do the opportunities to provide care, resources, and support for people facing severe craniofacial anomalies.

FACES has identified several potential strategic priorities that could be accomplished over the next three years, and we want to hear what you think about them.  Your feedback will ensure a robust plan that articulates our clients’ most pressing needs and then builds a plan to address those needs.

We invite you to take a few minutes to provide your input by completing this survey.  We are working with the Winkler Group, a strategic planning and capital campaign firm headquartered in Charleston, South Carolina, which will compile your feedback.

THANK YOU for being part of this important project!
Responses to this survey will be anonymous. However, respondents have the option to self-identify.
