Who should take this survey?

This survey is for adults (age 18 and older) who are working with WAVE's Family Services program or WISH's Child & Family Services program.

What will you be asked?

We want to hear your honest thoughts about your experiences with these programs. We'll ask you to share feedback about how services were delivered, whether services met your or your family's needs and expectations, and how we could better support families like yours in reaching their goals.

Are my responses private and anonymous?

Yes. This survey is anonymous. Please do not include your name or other personally identifying info. There are no right or wrong answers — we just want your honest feedback.

Who has access to the surveys?

WAVE & WISH works with an independent evaluator, Strategic Prevention Solutions (SPS), to collect and review survey responses. Both WAVE, WISH and SPS have access to the information, but individual responses are kept private and summarized before they are shared.

Why do we collect this information?

SPS reviews all survey responses and provides WAVE & WISH with a summary and/or report. This helps us answer important questions, like:

• Are families finding the services helpful?
• What benefits or changes are families noticing because of the program?
• How can we improve our programs to better support families?

A final report is also submitted to grant funders and other key partners to share valuable insights about what services, resources, and support strategies were most helpful to families – this helps inform future program development. Survey Your responses help us tell the story of what's working well and where we can grow.
