What is this survey?
Members of the lesbian, gay, bisexual, transgender, queer, questioning, intersex, asexual, and two-spirit (LGBTQIA2+, or LGBTQ+) community are being asked to define themselves – as opposed to letting others do it for us – by telling their life stories. This survey asks members of the LGBTQ+ community to define their experiences with health, safety, and accessing local services by reporting their experiences. ChangeLine (formerly known as Community Health Partnership / CHP) is working with local health providers and social service organizations to identify and address disparities and issues for the LGBTQ+ community in the Pikes Peak region. You can be a part in helping to better our community by completing the survey. It will only take 10 - 15 minutes and will be of substantial benefit for improving systems and creating a community where our LGBTQ+ neighbors can thrive.
Why should I fill this out?
First and foremost, because you are an important part of the LGBTQ+ community in the Pikes Peak region. By sharing your experiences, you will help contribute to the shared knowledge that healthcare providers, LGBTQ+ advocacy organizations, social service providers, and our community at large can use to improve community resources and public services.
Who should complete this survey?
Any LGBTQIA2+ person 18 years of age or older who lives in the Pikes Peak region. This survey should only be completed once per person.
Are there any risks associated with this survey?
- Some of the questions in this survey may make you feel uncomfortable, such as asking you to recall bad experiences. You may refuse to answer questions asked of you by not answering the question or selecting "prefer not to answer." There is also a content warning before the questions we believe may cause the most discomfort and you will have the option to skip this section entirely. You may stop participating by exiting the survey at any time.
- In case you need them, there will be a list of resources, including crisis hotlines, at the end of the survey.

all individual responses will remain confidential. The information gathered in this survey is for research purposes only, and the survey does not ask for personally identifiable information. We will only be reporting on the aggregate results of the survey, which means that individual responses will not be identifiable to the public, only proportions / percentages or total counts. This survey does not collect any Personally Identifiable Information (PII) such as names, emails, phone numbers and other information that can be directly or indirectly attributed to you. We ask for a voluntary zip code entry so that we can map out our responses and better understand the distribution of the data within the Pikes Peak Region.
All responses are recorded anonymously. Participation is voluntary and you may choose to not answer any questions or exit the survey at any time.
