Anticipated Annual Award Amount: Single award up to $90,000 annually.
Anticipated Project Dates: July 1, 2025 – June 30, 2026, with up to two possible one-year renewals.

Introduction

The Wisconsin Department of Health Services (DHS), Division of Public Health (DPH) announces the availability of funds through the Newborn Screening Program to increase access to newborn screening follow up services, clinical services, and genetic testing for members of the Plain community. 

There are several barriers for families that live in a Plain community that don’t exist for other residents of Wisconsin. Our goal is to improve the health outcomes of the rural populations living in Plain communities. This care might include confirmatory testing, special dietary treatment and nutritional counseling, genetic counseling, referrals and care coordination, and youth health care transition services.

This funding opportunity is for a single entity located in Wisconsin during the project period July 1, 2025 to June 30, 2026. The ongoing availability of funding is undetermined. With availability of funding, there is two possible one-year renewals.

Preference will be given to organizations that currently have a relationship and provide culturally appropriate services to the Plain community in rural areas of Wisconsin.

Project overview

The agency receiving this funding support will be expected, at a minimum, to perform the following project activities:
  • Identify Plain community patients with a confirmed diagnosis from patients referred due to newborn screen positive results.
    • Gather necessary information from various hospitals, midwives, and traditional birth attendants to establish an understanding of current newborn screening and follow-up practices.
    • Follow-up services could include confirmation testing and diagnostic services, special dietary treatment and nutrition counseling, genetic counseling, referral and care coordination, etc.
    • Document per patient data into REDCap for patients who received clinical care provided in clinic, by home visit, or ongoing management.
  • Make appropriate referrals to community and state resources (ie, Birth to 3 program, peer family support, etc).
    • Collect information on the needs of Children and Youth with Special Health Care Needs families in the Plain community and make referrals to the Children’s Resource Centers.
    • Collect feedback to better understand educational and support needs during and after screening and/or diagnostic testing.
  • Ensure Plain community patients have a youth to adult transition of health care plan in place.
    • Document per patient data into REDCap for patients who received clinical care.
    • Educate families on the importance of creating a youth health transition plan into adult healthcare.

Interested agencies should apply by answering the survey. The funded agency will, with the Newborn Screening Program, develop an approved budget and a comprehensive work plan outlining specific project deliverables after notification of award.
