Hybrid Training Program Application

GENERAL INSTRUCTIONS

This application is for both Personal Care Aide Training Programs and Home Health Aide Training Program who wish to apply to conduct their training program(s) using the hybrid model.
Please be advised that this application will require uploading files. You will not be able to save and return to this application at a later time. Please be prepared with all the supporting documentation at the time of submission.

Complete each question as identified in each part of this application.
Attach all pertinent documentation as indicated within the application.

Attestations are required for Hybrid Training approval and require the signature of the Operator/ Administrator. The PCATP Attestation is to be completed in application of a PCATP Hybrid Training Program. The HHATP Attestation is to be completed in application of a HHATP Hybrid Training Program.  Both Attestations must be completed if applying for both PCATP and HHATP Hybrid Training Programs.
