Approved In-Service Transcript Provider Request

If an approved clock hour provider (public school districts and accredited institutions only) wishes to use documentation other than the OSPI 1125 Form, submit the request form below to the OSPI Professional Certification Department. Once the transcript is reviewed, the school district or institution will receive notification of approval, and a copy of the transcript will be kept on file with OSPI. If changes are made to the transcript, the organization should provide OSPI with an updated copy.

Note: All School Districts, Charter Schools, and Compact Schools in Washington are automatically approved as clock hour providers. For more information about becoming a provider, visit the Professional Educator Standards Board Clock Hour page. 
Only currently approved clock hour providers may use this approved transcript service.

Approved Transcript Requirements

To be an approved transcript, the transcript must contain the following: 

  • The educator’s name 
  • WA Cert # (if possible, but not required) 
  • Course completion date
  • Number of hours
  • Course title 
  • Hour designations:
    • STEM, Equity, Leadership
    • Fundamental Course of Study and Para Subject Matter
  • Organization/Institution logo or seal
  • Signature from the organization designee, i.e., administrator, superintendent, clock hour manager, human resource manager etc.
