Request for Project Performance Reports.
In this survey, we ask for the HMIS project IDs and the date range for which you are requesting project performance reports. When reports are generated, they reflect data as it was in HMIS a day prior. Updates made on the day the report is generated will not be represented. For this reason, please wait at least a full day after making an update to the relevant HMIS data before making the request.
Please only request reports when you and your agency are ready to use them to learn about the project's performance. This allows us to work efficiently to help all agencies.
We may email you requesting additional details in order to complete your request.
Each provider agency may request reports twice in a quarter. Funder agencies may request once per quarter. Please see the All Chicago webpage for details instructions and explanations of the project performance reports.
