Health Q Consulting (HealthQ) have been appointed by Country SA PHN (Primary Health Network) to undertake stakeholder and community consultations related to alcohol and other drug (AOD) needs and treatment across all Local Health Network (LHN) locations in country South Australia.
You have been invited to participate in this survey as a member of the community that is employed with an AOD specialist service, and may or may not also have personal lived experience with alcohol or drug use.
All responses are considered confidential, and no direct attribution will be made or inferred in the way we report on the survey responses.
The survey will take approximately 10 minutes to complete. If you wish you can use the save and continue function and return to your survey later. The survey will close at 5pm 23rd May.
