Personal Care Aide Training Program Reapproval Application

 INSTRUCTIONS

                                                             GENERAL INSTRUCTIONS

Please be advised that this reapproval application will require uploading files. A "Save and Continue Later" button is available at the top of each page after the first page of the survey. This feature allows you to save your progress and receive a link via email to resume later. Please make sure to use the link sent to your personal email address to continue the survey. Please be prepared with all of the supporting documentation at the time of submission.

Complete each question as identified in all Parts of this application.
Attach all pertinent documentation where indicated within the reapproval application.
           
Please ensure the Operator’s Certification in Part III is signed by the agency’s director or administrator.

All programs are required to conduct monitoring on at least a quarterly basis and documentation of such must be included in the quality assurance minutes of the sponsoring agency. PCATPs are required to submit an annual evaluation report to the training entity’s governing authority. These annual reports must be included with the PCATP reapproval application.
