Public Review Survey: Draft Wisconsin State Aging Financial Plan and Intrastate Funding Formula

The new Wisconsin State Aging Financial Plan and Intrastate Funding Formula has been drafted and is ready for public review. This Financial Plan is part of the three-year State Plan on Aging. The State Unit on Aging invites you to provide feedback on the financial plan by completing this survey. Your input is both valued and appreciated! All feedback received by May 25, 2025, will be considered in crafting final revisions.
Based on your review of the Financial Plan, please rate how well or poorly it meets each of the following objectives:
1. Rate how well or poorly the Financial Plan describes the Older Americans Act requirements for distributing funds to aging agencies and programs in Wisconsin.
2. Rate how well or poorly the Financial Plan details the steps taken in distributing funds to aging agencies and programs.
3. Rate how well or poorly the Financial Plan explains the ways funds are targeted for services to older adults in greatest need.
