Health Q Consulting (HealthQ) have been hired by Country SA PHN (Primary Health Network) to talk to the community about alcohol and other drug needs and treatment across country South Australia.
You have been invited to participate in this survey as a person that has personal experience of alcohol and drug use, treatment, and care. This experience can be past or present.
We want to understand the needs of your community and the types of alcohol, tobacco and other drug services and supports you need.
We are asking these questions to understand the reasons people are accessing services. We also want to know how we can make these services better.
By sharing your experiences with us, you can help make the services better for other people.
You can choose to share as much or as little as you feel comfortable. Everything you tell us will be anonymous and kept confidential. The only time we would need to tell anyone else if there is a risk of harm to you or to someone else.
The survey will take about 10 minutes to complete. If you wish you can use the save and continue function and return to your survey later. The survey will close at 5pm 23rd May.
