CCAP Agreement Submission | 2025-2026

Hello and Welcome!

The California Community Colleges Chancellor's Office has designed the following College and Career Access Pathways (CCAP) Submission Form to assist the college districts/colleges in meeting the requirement outlined in California Education Code 76004 (c)(3):
 
A copy of the CCAP partnership agreement shall be filed with the office of the Chancellor of the California Community College and with the department before the start of the CCAP partnership. The Chancellor may void any CCAP partnership agreement it determines has not complied with the intent of the requirements of this section.

As outlined in Education Code 76004 (Link Only):
 
The governing board of a community college district may enter into a College and Career Access Pathways (CCAP) partnership with the governing board of a school district, a county office of education or the governing board of a charter school for the purpose of offering or expanding dual enrollment opportunities for pupils who may not already be college bound or who are underrepresented in higher education, with the goal of developing seamless pathways from high school to community college for career technical education or preparation for transfer, improving high school graduation rates, or helping high school pupils achieve college and career readiness.

As used in this section "high school" includes a community school, continuation high school, juvenile court school, or adult education program offering courses for high school diplomas or high school equivalency certificates.

1. Please select only one document to be submitted as defined by the Chancellor's Office:
  • Fully Executed: New CCAP Agreement signed by both districts, approved by both governing boards and included the appendix of courses as outlined in Education Code 76004.
  • Updated Appendix: Changes to the original CCAP Agreement Appendix/Course list, such as additions or deletions of course(s).
  • Amendment: Changes to an originally submitted CCAP agreement, such as additions, deletions or modifications within the subsections of the original agreement.
Please Note: When uploading documents, please ensure they are fully executed. Agreements and/or Appendices that are in progress or not fully executed will not be accepted. *This question is required.
