The Stafford County Healthcare Coalition - made up of members from the Stafford County Health Department, Stafford County Hospital, K-State Research and Extension 21 Central District, and Stafford County Healthcare Providers - want to hear from you regarding community health needs and concerns. The following survey has been created in order to understand the state of health services in Stafford County, learn about how community members use local health services, and to gather feedback on how the local health systems can improve. You will be asked basic questions about yourself, your health care services, and the health of the community in Stafford County. You may skip questions you do not want to answer. The survey should take 10 to 15 minutes to complete. You must be over the age of 18 to complete the survey.
Participation in the survey is voluntary and your responses will be anonymous. Your individual answers to this survey will be confidential. If results are published from this survey, only group results will be discussed. Any information that might identify you as a participant will not be shared. While your participation is voluntary and confidential, all community input is valued.
This survey was developed by the Center for Applied Research and Evaluation (CARE) at the Community Engagement Institute of Wichita State University and the Stafford County Healthcare Coalition. All survey responses will be combined into one report by staff at the Wichita State University (WSU) Community Engagement Institute (CEI).
