The Wisconsin (WI) Department of Health Services (DHS) Primary Care Program has issued a request for applications (RFA) to provide interested parties with information on preparing and applying for the Health Care Provider Training Grants. Organizations interested in applying must complete this online application to be considered for the award. All questions must be answered.
