The Office of Superintendent of Public Instruction (OSPI) Special Education division annually reviews approved nonpublic agencies (NPAs) authorized to provide special education services to students from public school districts in Washington. Approved NPAs are required to submit an annual renewal application for each authorized location/entity. Completion and submission of this form on an annual basis fulfills this requirement. Failure to submit the annual renewal application form to OSPI by June 30th may result in a review of NPA status and possible removal from OSPI’s list of approved NPAs.
Before completing this application, please prepare electronic copies in advance of:
- A roster of students (name, grade/age, district) placed at the NPA by Washington public school districts.
- An aggregated summary report of attendance and discipline for students placed at the NPA by a Washington state public school district during the past school year.
- An aggregated summary report of incidents of restraint/isolation and critical incidents for students placed at the NPA by a Washington state public school district during the past school year.
- An updated Special Education Personnel Record (or similar document) listing only any new credentialed special education or related services personnel that have been hired within the past school year. (For out-of-state licensure only - please include current copies of credential/license).
- A list of ongoing staff training, onboarding, and professional development topics, including the dates offered, during the past school year.
