Abundance Foundation Application

What are you applying for?

Please read below for eligibility requirements before proceeding to the application(s):

1. We are based in Fort Collins Colorado and currently serve Northern Colorado.

2. If you are applying for Men's sober living, Women's sober living and/or a scholarship you must either already be working with a Recovery Coach, or you will need to apply for one now.

3. Colorado Medicaid is required to work with a Recovery Coach.

Important Notice Regarding Services

Due to changes in Medicaid policies, individuals may receive PHP or IOP services through another organization while receiving peer coaching through Abundance Foundation. However, peer coaching services may only be provided by one organization at a time.

Choosing Abundance Foundation for peer coaching means agreeing not to receive peer coaching services from another organization simultaneously.
