Confidentiality
All information shared will be kept confidential and your privacy will be protected except as required by law. The information collected will be used in the production of internal reports shared with Health Information Pathway partners, and no identifying information will be released or published.
Any records containing information from your application will be stored securely for one year after the project completion. After a year post completion, the records will be destroyed. If you are sending personal information by email, please be aware that electronic communication is not always secure.
Please do not share personal health information in this form or in your correspondence.
Ontario Health is designated an “institution” by the
Freedom of Information and Protection of Privacy Act (FIPPA),1990 and is collecting your personal information pursuant to
FIPPA and the
