Required is a narrative description of the policy and/or initiative for which the grant award is being sought. The narrative should total not more than 1,000 words and answer the following questions:

What is the name/title of the existing policy and/or initiative you are submitting for consideration?
What are the stated objectives of the policy/initiative, both general and specific?
Is the policy/initiative citywide or targeted to specific communities or groups? If targeted, please describe the communities or groups.
For the problem(s) being addressed, what evidence prompted the policy to be implemented and/or the initiative to be undertaken?
What are the components of the policy/initiative? (This is your basic description of how the policy/initiative operates.) Are there features that you regard as innovative?
How long has the effort been underway?
An important component in the judging of grant applications submitted is evaluation. To date, what evidence of the impact of the policy/initiative and related actions have you obtained, and what does it show? (This may include any internal or external evaluations, statistical evidence, feedback from target communities, etc.)
What other factors do you believe to be relevant in the judging of the policy/initiative you are proposing for an award?
If grant funds are awarded, 1) how will they be applied to the continuation, enhancement, expansion, promotion and/or other purpose in support of your policy/initiative, and 2) what impact would the additional grant funds be expected to have on reaching your policy/initiative objectives?

Note that, in addition to serving as the source document for the information on which the panel of judges will base grant award decisions, a summary of this narrative will be posted on the Conference of Mayors website to provide all mayors with information on what their colleagues have found to be effective reforms that are worthy of support and potential replication. *This question is required.

Final Report Requirement

As a condition of receiving an award, grant recipients will agree to provide a final report within 12 months of disbursement of the funds. This report, not to exceed 1,000 words, shall describe:

areas of the policy/initiative in which the grant funds were invested and the impact of that investment on its scope and/or level of effort;
impact of the grant investment on the objectives stated in the grant application;
modifications to the policy/initiative made (or recommended) based on experience gained in the period following the investment of the grant funds.
