Be assured that your individual responses will remain confidential, and any reporting or references to survey results will be presented in aggregate form only.
Before we get started, a few helpful hints...
- Once you've answered all the questions on the screen, click the "Next" button to advance to the next screen in the survey. Please DO NOT use the "Enter" key to submit your responses, and DO NOT use the forward and back buttons on your browser's navigation bar to move forward and back in the survey.
- If you run into difficulties completing the online questionnaire, please contact the survey administrator: Paul Stewart at pstewart@brammresearch.com.
- If you are interrupted while completing the survey you may click on the "Save and continue later" bar that appears at the top of each page, where you can send your saved responses to the email address of your choice. Please be aware that the message containing your personal link might get diverted to your spam folder. So please check there if the email does not appear in your Inbox.
- Questions marked with a red asterisk* are required and must be answered.
