MN Farm to School Institute Application 2025 - 2026


Please read the instructions below and review the Farm to School Program Assessment Guide before filling out this form. 

Complete one application per team. Individuals may not apply.  A view-only copy of the application questions can be found here.

Applications must be received by Friday, March 14th, 2025. 


We encourage and seek diverse teams that contain members of different racial and gender identity, experience levels, and program roles.  School or district applicants must be located in Minnesota.

Individual School-Based Teams: Teams must be composed of 4-5 individuals representing various areas within the school and community.  
Teams must have representation from the following three roles: 
  • Administration
  • Teachers
  • School nutrition/food service
  • Additional team members could include: school staff, farmers, school board members, community partners, etc.

School District-Wide Teams: Teams must be comprised of 5-6 individuals representing various areas within the school district and community.  
Teams must have representation from the following three roles: 
  • District-level administration
  • Teachers
  • School nutrition/food service 
  • Additional team members could include: school staff, farmers, school board members, community partners, etc. 
In your application, please identify one or two particular schools within your district in which you will focus efforts on implementing your Farm to School Action Plan.  This can serve as an example for other schools within the district to guide them through this process.

We recommend completing your responses to the application narrative questions below in a separate document. You can copy/paste your responses to the application when you are ready to submit.

Please review the School Team Evaluation Criteria for more information on how the evaluation team will select teams for the Institute.
