Welcome to the Eviction Legal Services Program Screening Form

Thank you for reaching out for assistance. This form is designed to assess your eligibility for eviction legal services through the City of Phoenix. These services aim to provide tenants with legal representation, mediation and post-eviction services.

To qualify for eviction legal services, applicants must meet the following criteria:
  1. Residency: You must currently reside in Phoenix, Arizona or receive City of Phoenix water services.
  2. Eviction Risk: You must be facing eviction or already have been evicted.
  3. Income: Your household income must be at or below 80% of the Area Median Income (AMI) for Maricopa County.
If eligible, services may include:
  1. Legal representation at any Phoenix serving Justice Courts and/or the Superior Court eviction proceedings;
  2. Mediation (outside a courtroom), to include negotiations with your landlord; and
  3. Post-eviction legal services.
