This survey is being conducted by the Northern Kentucky Health Department on behalf of organizations who serve the community. The results of this survey will help us improve the health of people who live, work, and play in Boone, Campbell, Grant, and Kenton Counties.
Your answers will be combined with many others; we will not share your individual responses with anyone. Your answers are completely confidential, and your participation is voluntary. Please answer each of the following questions to the best of your ability.

You must be at least 18 years old to complete this survey. Once complete, you will have the opportunity to enter for a chance to win one (1) $50 gift card (merchant varies) at the end of survey! There will be thirty (30) winners chosen at random.
Survey results will be available and shared broadly in the community within the next year.
