LPHA Annual Survey 2025

LPHA Information

This is the annual survey that fulfills the deliverable for the OPHP Local Planning and Support funding (i.e. per capita funding). This is the Task Order Contract that all LPHAs receive and is allocated via a funding formula. In this survey we ask about the Core Public Health Services, both the Foundational Services and Foundational Capabilities.

In this survey there are questions about workforce, funding, capacity, and services of LPHAs.  All questions will help to make the case for public health through point in time and longitudinal, and quantitative/qualitative information.

Information Sharing:
  1. Some individual survey responses will be shared with CALPHO to better understand services provided and financial billing.
  2. A public facing information/dashboard will be created and have some individual level and some summary information.
Please note: there is a link to the Maternal and Child Health questions at the end of this survey.  We have the MCH survey as a separate survey link because we know in some cases it will be a different person responding.
