2024-2025 SJC WPI Program

As part of the 2024-2025 St. Johns County Wellness Premium Incentive (WPI) Program, participants can complete one of the approved health screening services to become eligible for the WPI. All enrolled employees and spouses are eligible for this program discount. The discounted premiums will be applied in the 2026 plan year (effective 1/1/2026).

If you are a newly enrolled member (new hire, qualified life event, etc.) to the plan, your visit must occur within the last 12 months. The credit will be applied as soon as administratively possible following receipt of your submission.
If you are a currently enrolled member, your visit must occur within the program dates of October 1, 2024, through September 30, 2025. Any submissions after the program deadline will not be eligible for the credit in 2026.

After submission, you should receive a confirmation email to the email address entered. Be sure to check your spam/junk folder as well. Please keep your confirmation email as a verification of your submission.
