Help shape a happier, healthier Yampa Valley!
Northwest Colorado Health, UCHealth Yampa Valley Medical Center, Memorial Regional Health, The Health Partnership, and the Public Health departments of Routt and Moffat counties have partnered to launch this survey and we are asking for your input in a Community Health Needs Assessment (CHNA). Partner organizations may utilize a subset of the survey responses to inform their respective CHNAs based on their mission, service offerings and populations served. This survey helps identify the most important health needs in Routt and Moffat Counties.
A CHNA gathers information from two sources:
- Your feedback as a community member.
- Health data from places like the Colorado Department of Public Health and Environment (CDPHE).
By combining this information, we can find ways to improve local health services and focus on what matters most.
Your input helps create plans for local needs, such as:
- Adding translation services to make health care easier to access.
- Reducing alcohol-related car crashes through prevention programs and community partnerships.
Survey Details:
- Anonymous & Voluntary: Your responses are confidential and combined with others for analysis. This survey is intended for residents of Routt and Moffat County who are 18 years of age and older.
- Timeframe: Open from February 1 - February 21, 2025; takes about 13 minutes to complete.
- Incentive: Enter to win a $250 gift card by sharing your name and email after completing the survey (optional). The winner will be contacted within two weeks of the survey closing.
