Welcome to the Dairy Producer Grant Reimbursement Form!
To ensure a smooth and timely reimbursement process, please carefully follow the instructions below:
1. Complete this reimbursement form within 30 days of the event date.
2. Upload up to 10 original, itemized receipts from stores or vendors. Handwritten receipts are not accepted. At least one receipt must be uploaded to submit the form.
3. UDIM will issue a reimbursement check for approved expenses supported by original itemized receipts within 30 days of receiving the completed reimbursement form.
