TEP Nomination Form: Cerebral Palsy Management for Adults

Instructions: 

Applicants/nominees must submit a completed and signed TEP Nomination Form along with the following documents:  

• A brief statement of interest (not to exceed 1 page) highlighting experience and expertise relevant to the TEP subject area, including: 

  • Basic understanding of consensus building for clinical subjects 

  • Expertise in Cerebral Palsy management, with descriptions how applicant meet those criteria 

  • Understanding of various clinical guidance documents and interpretation of evidence.  

  • Please also describe how you are working with Adults with Cerebral Palsy in your current clinical practice setting 

  • Please describe your published research or national level presentations on the field of Cerebral Palsy.  

A curriculum vitae (CV) or a summary of relevant experience (including publications) for a maximum of 10 pages.
