District Application



Thank you for applying for District Recognition.
This application is for districts that have training, coaching, and evaluation in place to support schools in their district implementing SW-PBIS.

There are 2 levels of recognition at the district level. The first is to highlight districts that are in progress towards sustaining implementation and have multiple schools measuring fidelity annually.  The second is to highlight districts that have demonstrated exemplar district capacity with 60% of schools implementing SW-PBIS with fidelity as measured annually.

Eligibility Requirements:

1. Active Implementation of PBIS in at least 2 schools as evidenced by current action plans and data collection within the past 12 months in schools
  • For districts with more than 10 schools total, at least 6 must have been trained in PBIS and actively implementing
  • For districts with fewer than 10 schools total, 60% must have been trained in PBIS and actively implementing
2. Submit an application demonstrating district capacity supporting schools implementing PBIS.  The application includes the following sections:
  • Your information
  • Coordination
  • Training
  • Coaching
  • Evaluation

Please submit your application by April 2025. Districts who meet Sustaining Exemplar status will be notified as soon as reviewal process has been completed.
