2025 Exemplar PBIS Application Cohort

Thank you for applying to become a PBIS Sustaining Exemplar School. 
This application is for individual school recognition for those that were cohort trained by the state.

The Save and Continue survey toolbar adds an option for you to enter your email address to receive a link to return to your survey later. 

The toolbar will be added to all pages after the first page of your survey.

The eligibility requirements for this application include:

• Completed 2-year PBIS training through a cohort with the Minnesota Department of Education or one of the Regional Implementation Projects
• Demonstrate fidelity of implementation as measured by the Tiered Fidelity Inventory (TFI) or the Benchmarks of Quality (BoQ) within the past year. 
  • Overall BoQ scale score of 80 percent or above
  • Overall TFI scale score of 70 percent or above

Deadline for applications to be included in recognition ceremony is April 2025. Individual schools who meet Sustaining Exemplar status will be notified as soon as reviewal process has been completed.
