Expecting or Parenting Foster Youth Survey

Consent to Participate

Title of study: Research Study on Expecting and Parenting Foster Youth to Increase Permanency with Caregivers

What are you being asked to do?
You are being asked to complete one online survey. This survey’s purpose is to get information from young people who are expecting a child, have a child, or parented a child while in foster care in South Carolina. We will use the answers from this survey to help Fact Forward in creating a training (or class) for foster parents or caregivers that support expecting or parenting teens. This training will help foster youth stay with their foster caregivers

Who will be participating in this focus group?
We are asking for 20 young adults aged 14 to 24 who are expecting a child, have a child, or parented a child while in foster care in South Carolina to take this survey.  If you are under the age of 18, you will need your parents’/guardian’s consent first.

What if I do not want to take this survey or change my mind?
Taking this survey is optional. You may choose not to take the survey. If you do agree to take the survey, you may choose not to answer some or all the questions on this survey and you may stop at any time. The survey should take about 15 minutes to complete.

Will my answers be kept private?
Your answers will be private, and no one will know they are yours. We will only ask for your name and email address at the end of the survey so that we can send you a gift card for your time taking this survey. 

There may be some risks from taking in this survey.
  • Discomfort with questions: Some people feel uncomfortable when answering questions about sexual and reproductive health and other sensitive topics.  Though it is always better to have fully completed surveys, you do not need to answer any questions that make you feel uncomfortable.
  • Loss of privacy: There is a risk of loss of privacy. This risk minimized in the following ways:
    • Personal information will only be asked so that we can send you an e-gift card for completing the survey. Your responses will not be connected to your name and will remain private.
    • Study data will only be seen by Fact Forward staff through a password protected account.
  • Unknown Risks: There may be other risks that we cannot guess right now. We will tell you about any new information which may affect your decision to participate in this study.

Will I get paid to participate?
You will get a $50 e-gift card sent to your email address for completing the survey. Although you will not benefit directly from this survey, it will help Fact Forward better understand what young people need.
