Cessation Intervention Sustainability Assessment Tool-2024

This assessment should take about 10 minutes to complete. 

Purpose
The American Lung Association has developed this interactive assessment tool for tobacco control program teams and their partners to help identify organizational strengths and targets to further opportunities for improvements in planning for sustainability. This tool is available for use by all tobacco control program staff and other public health professionals and their community partners that are working towards health system changes that integrate evidence-based cessation interventions.


Outcome
After completing this assessment, you will be sent a PDF copy of your scored assessment to the email you provided. Your assessment score will help identify strengths and targets for increasing sustainability of current cessation interventions within your organization. 

Availability of Assessment
You are welcome to re-take this on-demand assessment periodically as your organization or program periodically assesses its capacity and sustainability practices to enhance efforts towards comprehensive cessation efforts. Your email address and other identifying information will not be shared or used outside of the scope of this assessment.
