CSMH Survey: Youth and Family Survey

Overview: The Child Health and Development Institute is completing a school mental health landscape analysis to better understand current efforts nationally and statewide around supporting student behavioral health and well-being. By completing this survey, you are providing your input and recommendations on what is working and what can be improved/strengthened. Once all the surveys are completed, a report with recommendations on how schools can strengthen behavioral health and well-being will be created and disseminated.

We’ll be asking you about Behavioral Health. When we ask about behavioral health, we mean the broad spectrum of concerns related to emotional and behavioral needs of students, including well-being, resilience, mental health, and substance use prevention.

Directions: Please complete this survey to the best of your ability. Personal or identifying information will not be used or shared in any reports or findings. You are free to skip any question, but if you can, please fill out the entire survey. This survey should take approximately 10 minutes to complete.
