Introduction and Background

Welcome to CHRISTUS Health’s Community Health Needs Assessment Survey.
This survey will take around 10 minutes to complete. We will ask you questions about the health needs of your community. The information we get from the survey will help us:
  • Identify health and social issues that affect the people in your community.
  • Understand the needs of your community.
  • Work together to find a solution.
     
The survey is voluntary, and you do not have to participate. You can also skip any questions you do not want to answer or end the survey at any time.

The answers you give are very important to us. Your answers will be private (we will not know who gave the answers) and we will protect the information you are giving. We will not share your personal information or survey answers with anyone outside CHRISTUS Health.
