License Record Updates

Introduction

The following survey is to be used by Adult-Use Cannabis licensees to gather information to begin the process to request an amendment to the licensee’s license record. Please note the submission of this survey does not constitute approval for any updated information or amendments to the licensee’s record. Office approval is required for any update to take effect and in some instances Board approval is required before a change can take effect (e.g. location change).

The Survey includes four areas to request amendments or updates to the license record.

1) License Overview – this section will allow requests to update the following information:

Legal Name (please note – entity type changes are not permitted)
Doing Business As (DBA) Name
Business Website
Only for Sole Proprietor Licenses requesting entity type change to LLC or Corporation – collection of certain information to request an entity change.

2) Contact Information – this section will allow requests to update the following information:

License Primary Contact Information
Person Upon Whom Process can be Served
Business Mailing Address
Office / Record Keeping Address

3) Location Change Request - this section will allow requests to update the following information:

Operating Address (where cannabis operations are performed)
Certain Cultivation Tier Requests (if applicable)

* Please note this survey can only process one location change request at a time. If you would like to request the addition or removal of multiple locations, a separate survey will be required for each request.

4) Notification of Ownership and True Party of Interest (TPI) - this section will allow requests to update the following information:

At this time, the Office is not accepting ownership change requests or updated TPIs from licensees. However, the Office has issued guidance Interim Guidance for True Party of Interest Changes which outlines certain actions licensees can take now before the amendments system is available.
This section of the survey is used to sign-up to be notified by the Office when the amendment system is available to be able to enter any changes which may have occurred while following the Interim Guidance for True Party of Interest Changes.
