2025 Emerging Leaders Program Application

The Emerging Leaders Program is for community-based behavioral health care providers in Wisconsin. It is not a leadership training for DHS employees. Learn more about this program.

Describe your experiences that demonstrate your leadership in your community. Include the strengths you bring with you, areas of opportunity for personal and professional growth you want to improve, and what you hope to gain from participation in this program. Include barriers or challenges you have faced and how you feel this program could assist in your ability to move into a leadership role and be able to give back to your community. Include your personal, professional, and community experiences specific to underserved communities, communities facing barriers in accessing and using mental health and substance use services based on geographic location, religion, sexual orientation, gender identify, racial or ethnic status, or other special needs.

Please upload a letter of support from your employer/supervisor that indicates their support of you participating in this program, including approval to attend all of the virtual/online events from 9 a.m. to 12 p.m. on the following dates: January 21, February 4, February 18, March 4, March 18, June 10, June 17, September 10, September 11. 

If self-employed, a signed letter from yourself committing to the dates and times is sufficient.
