Mahi Mihinare Anglican Action (MMAA) 2024 Survey

Introduction and Consent

Hello, 

Mahi Mihinare Anglican Action has teamed up with Huber Social, a group of experts in measuring the impact of programs, to see how much of a difference Mahi Mihinare makes on people like you. We want to ask you a few questions, where your answers will be kept private and confidential, so we won’t know that they were your answers. The survey will ask you about how you feel in general and things about wellbeing. It will take around 10-15 minutes to finish, and we would really appreciate your answers. Your privacy 

In this survey, you will not be asked to provide your name, number or any other contact information; all survey results remain private and no individual results will be shared. There are two questions at the end where you have a chance to give feedback in your own words. If you answer these questions, Mahi Mihinare might see them, but they won't know it's from you. Your personal information won't be shared with anyone else without your permission unless it's required by law. 
Your information will be kept safe in the Huber Social Wellbeing Database. Besides this project, your information might be combined with other data to learn more about the well-being and needs of people. This data may be used by Huber Social or companies that have a special data-sharing policy with Huber Social which has strict privacy rules. At the end of the survey there will be two questions that will ask you to provide written feedback. Written answers may be shared back with the company you are a part of, however they will not be able to tell that I wrote it. How to complete this survey 
Please read each question/statement carefully. If you do not wish to answer a question, you can skip it. If you do not understand a question, please reach out to your key worker, or you may contact Robert at robert@anglicanaction.org.nz for help, otherwise you may skip the question. Please take breaks as needed but remember to keep the browser window open, otherwise, the survey will end. You may stop the survey at any time. If you cannot complete this survey in a safe environment, please do not do it. If you would prefer to, you are welcome to go to a local Mahi Mihinare office and complete the survey there.

This study has been approved by the Human Research Ethics Committee at Huber Social. They checked that it's appropriate and fair. The ethics reference number is: 16941428059.

  • I have read or had someone read to me the information about this survey. I also had a chance to talk about it with the person helping me. 

  • I understand what we will do in this survey and how long it will take. Any questions that I have had, have been answered so that I understand. 

  • I am over 16 years old and I can agree to take part in this survey  

  • This survey is optional. I can stop anytime if I want. I understand that even if I stop, the data that I have already entered will be submitted. 

  • The results of the survey might be shared with others, but they won't use my name or any personal details. My answers will stay private.
