LGBTQ+ Health Needs Survey

Introduction

Thank you for your interest in our LGBTQ+ health needs assessment! This assessment is sponsored by the Los Angeles LGBT Center. Your participation is very important to help us learn about the health needs and experiences of queer people who have ever experienced girlhood or womanhood. 

Your answers will help us better understand LGBTQ+ individuals healthcare needs and the challenges you might face in accessing care. We appreciate your openness, and we will keep your answers private. Your answers will be kept confidential. You are not required to answer every question. Each question has the option “Prefer not to say.” You may stop the survey at any time without penalty or getting in trouble. Thank you for helping us learn more!

The first 2,000 eligible respondents will receive a $15 e-gift card, but please note that we may not be able to give the gift card if the survey isn’t fully completed. To receive a gift card, you must provide an email address. We will only ask for your email address after completing the survey. Your email address will not be linked to any of your responses. It will be asked separately in a new form after completing this survey.
 

You may not be eligible for any of the following reasons: 

  • The survey isn’t fully completed
  • The survey doesn’t pass the security checks which protect our data from fraudulent responses and bad faith actors
  • You do not provide your email address after completing the survey.
