North Carolina immunization law (G.S. 130A-155.1(c)) requires all public, private and religious colleges and universities to submit an immunization report annually to the Department of Health and Human Services. The purpose of this report is to ensure that all students in attendance are protected against vaccine-preventable diseases and to determine the number of students who meet state immunization requirements. It is the responsibility of the college or university registrar to ensure that accurate and complete data is submitted by the legislatively mandated date.Executive Order 319 extended the deadline for compliance with immunization requirements due to Hurricane Helene.
The period from the first day of attendance through October 31, 2024 will not count toward the 30-day grace period for students to comply with immunization requirements.
This report will be open for an extended period and is due by midnight of December 31, 2025.
Please complete the following survey to record the immunization status of new undergraduate and graduate students enrolled in your school. The required survey takes approx. 15-30 minutes if the worksheet has been completed ahead of time. Please fill out that worksheet before starting this survey.
