2024-2025 Child Care Immunization Report

Please be sure to:

Submit a report even if no children are attending your child care center or your child care center has closed. This will ensure your center meets the reporting requirement and is not considered delinquent.

Executive Order 319 extended the deadline for compliance with immunization requirements due to Hurricane Helene. The period from the first day of attendance through October 31st, 2024 will not count toward the 30-day grace period for students to comply with immunization requirements. The 30-day grace period will commence November 1st, 2024. The new exclusion date is December 1st, 2024.
This report will be open for an extended period and is due by midnight of January 15th, 2025.
Failure to submit an annual child care immunization report per G.S. 130A-155 will be reported to the Local Health Director for your county.
