2024-25 DoD Allocation Adjustment Form

Please use this form to request DoD allocation adjustments for the 2024-25 SY. Requests will be processed after the 10th of each month. If your request is submitted after the 10th, it will be reviewed the following month.

Please note: This form is to be completed by the Designated Official only (Program or Ordering Contact per CNPweb). The first review of the 2024-25 SY will be after September 10th. June 10th will be the last and final date for DoD adjustments for the current school year, so please plan accordingly.
